CARDIOLOGY PROCEDURE NOTE

ASSISTANT: 

PREPROCEDURE DIAGNOSIS: Persistent atrial fibrillation. 

POSTPROCEDURE DIAGNOSIS: Sinus rhythm. 

PROCEDURE PERFORMED: Electrical cardioversion. 

COMPLICATIONS: None. 

CLINICAL PROFILE: 


I reassessed the patient immediately prior to the procedure and found
him/her an acceptable candidate for moderate sedation: 


SUMMARY OF PROCEDURE: After risks, benefits, and alternatives were explained to
the patient, informed consent was obtained.  The patient was brought to the post
anesthesia care unit in a fasting and non-sedated state.  Sedation was performed
by the anesthesiology team.  Defibrillator pads were applied to the left
parasternal region and left subscapular region.  200J of biphasic synchronized
energy were delivered with conversion to sinus rhythm.  The patient
tolerated the procedure well without any apparent complications.  He was
monitored in stable condition until discharge.

Discussed with patient/patient representative the details of his/her
diagnosis, management, treatment plan, possible complications, discharge
orders and follow up care in which the patient and/or representative
verbalized understanding.
