EP PROCEDURE NOTE

PROCEDURE:
The risks and benefits of the procedure were explained to the patient in full,
and in simple terms.  Alternative therapy options were given, including no
procedure.  The risks of the procedure include, but are not limited to: pain,
bleeding, infection, perforation of vessels or heart, pericardial
effusion, unstable heart rhythm, and death. The risks of blood transfusion
were discussed, and include fever, transfusion reaction, and infection. All
questions were answered, and the patient voices understanding.  The patient
acknowledged the risks of the procedure, and still wanted to proceed. 

The patient was brought into the Electrophysiology Lab in a fasting and
hemodynamically stable condition. 

A formal time out for patient identification and review of procedure was
performed.  The left chest wall was vigorously washed, prepped, and sterilely
draped. The chest wall was anesthetized with lidocaine. 

In incision was made in inferior portion of the left chest in an oblique
angle. Using the supplied tool, we then tunneled underneath the skin. 
The loop recorder was then "injected" into the area with the aid of the tool. 

We placed one Vicryl 4-0 subcutaneous suture to close.  A steri-strip was then applied to
the incision. 

The patient was then transported back to a monitored bed.

Versed

Fentanyl 



EQUIPMENT USED:
1.   Boston Scientific Lux-DX II+ M312, serial #


PROGRAMMING:
Tachycardia: 170 bpm 
Brady:  40 bpm x 5 seconds
Asystole:  3.0 seconds (day time) 5.0 seconds (nocturnal) 

AFib: 10 minutes 

Atrial Tachycardia: 120 bpm x 4 hours 


PLAN:
1. Wound Care: No bathing/showering x 48 hours.
2. Follow up with Pacemaker Clinic in 2-4weeks.
